How do I use the benefit for hospital referrals?
A listing of the 5 closest hospitals for the member's zip code is included with each membership kit. Members may access hospital listings online at by clicking here.
Members may call our toll-free number to obtain additional hospital referrals between the hours of 7 a.m. and 7 p.m. Central Time, Monday through Friday, and 8 a.m. to 5 p.m. Central Time, and Saturdays.
How do I use the benefit for hospital referrals?
Members simply present their card at the hospital for a discount off the cost of services.
If the procedure amount will be more than $1,000, you must pay $1,000.00 at the time service is rendered. You will then receive a billing statement detailing your savings and the amount you must pay within 30 days of service. For procedures less than $1,000, you are required to pay a portion of the full amount determined by the hospital at the time service is rendered. The balance will be due within 30 days of you receiving the re-priced billing statement.
Does the medical plan include my family?
